hello

"Repetitions will decrease every 2 weeks in the following order: 15 reps for 2 weeks Þ 10 reps for 2 weeks Þ 5 reps for 2 weeks Þ then continue with your 5 rep max for 2 weeks....."

This is a cut and paste from hst notes.. At the end here it says; continue your 5 rep MAX for 2 weeks(or negative). Dose this mean; lets say 120 kg bench press for two weeks not using decreasement(120 kg every exercise) OR starting the 5 rep as the two weeks before?

It means working with the last weight of your 5 rep phase.
Example
Week One of 5's
110, 115, 120,
Week two of 5's
125, 130, 135,
Continueing with 5RM
135, 135, 135,
135, 135, 135,

If you can raise the weight during the extension feel free, if you can't no big deal.
