phs and hst...questions about faq from blade

hi, ive got some problems with understanding the tips from blade...ok to summarize:

while on phs:

dont do sd
dont do 15's
increase volume (sets per muscle)
increase frequenzy
dont increase wheight instaed increase reps (although I think this is hard to find out how many reps to do that you dont go to fast to your limite)
2 on / middle 10's, start 5's
4 off / rest

is that correct? some more tips?

-are 4 off necessary if I am using a anti-estrogen?
-what about increasing volume and frequenzy? any tips? I was thinking about 5-6 workouts per week and up to 4 sets per muscle?
-he wrote somewhere just go up from your 15's and increase wheights in each session until you hit your 5s? should you dont follow your plan and instead just increase wheight every workout and therefore drop reps when necessary?
-2on 2off is a good schedule? somebody told me to do 4 week cycles because it takes 10 days for phs to hit in...
