Neck pain

Guys, I need some opinions. A while ago, I got this brutal muscle pain in the back of my neck, while I was doing chinups. It's a pretty intense pain, and it always ends up giving me a harsh headache too for the rest of the day. Anyways, now it's come back, and its happening almost every workout. I'm thinking I should take a break again... but what good is it going to do if it just comes back? What could I be doing wrong? I must've pulled a muscle or something... but I'm thinking maybe I'm clenching my teeth too hard when im lifting... or maybe im lifting my head up while im benching and its putting too much stress on the neck? I'm not even sure I'm doing those things, but could they be possibilities? I'm choked... any advice guys?
 
Do you push your chin forwards when you do chin ups?
