Yesterday I established my 15 rwp maxes.

1. If I understand this correctly, I subtract 5 lbs for every workout in the 2 week period and on the sixth workout I will be at the numbers established yesterday. On my next cycle, how do I arrive at my new max?

2. Is a max considered working to failure?
 
1. 5 0r 10, depending on the movement
For your next cycle just add 5lbs to each rep max for small muscle groups and 10 to 15 for larger groups.

2. Yes and no, some hit failure when working with each RM, but it is not necessary you can simply stop when you know your next rep wouldn't be finished. But if you are into working to failure and you can recoup enough to maintain strength for your next workout then by all means go for it.
